Application of esophageal echocardiography in artificial chordae mitral valvuloplasty

Recently, researchers from the Department of Ultrasound, the First Affiliated Hospital of Nanchang University published a paper to explore the value of esophageal echocardiography (TEE) in artificial chordae implantation for mitral valvuloplasty. The study pointed out that TEE in the artificial chord implantation mitral valvuloplasty is helpful to determine the mitral valve prolapse site, the reference length of the artificial chordae and evaluate the surgical results. The article was published in the 33rd issue of the Journal of Modern Chinese Medicine in 2014.
Thirty-two patients with mitral valvuloplasty who underwent mitral valvuloplasty were treated with preoperative TEE to observe the mitral valve prolapse and measure the artificial chordae reference length, which was compared with the standard seen during surgery. Postoperative TEE was used to observe the degree of mitral regurgitation and to measure the height of the anterior and posterior valvular rims.
132 patients underwent preoperative TEE diagnosis of prolapse of 43 segments of 34 leaflets. During the operation, 47 segments of 35 leaflets were prolapsed. The diagnostic accuracy of TEE was 97.14% and 91.49%, respectively. 2 The actual length of artificial chordae was shorter than that of preoperative TEE. The deviation ranged from 0.1 to 2.7 mm and the mean deviation was (1.24±0.64) mm. However, all patients had good postoperative results and the valve could be opened and closed. Among the 332 patients, there were 6 patients with moderate reflux before operation and 26 patients with severe reflux. There were 28 patients with mild reflux and 4 patients with mild reflux. The mitral regurgitation was better than before operation. Significantly improved; the height of the anterior and posterior valvular rims was >8 mm, with an average of (10.30±0.43) mm, and the valve alignment was good.
