Theophylline, also known as 1,3-dimethylxanthine, is an important member of the Astragalus family. It was first extracted and recognized by German biologists in 1888. Subsequently, theophylline was artificially synthesized and used as a drug, and its main clinical application was diuretics. After more than two decades of application, theophylline has been found to have a very significant therapeutic effect on asthma, and has since been developed for the treatment of respiratory diseases such as chronic obstructive pulmonary disease (COPD) and asthma and has been used to date.

Studies have reported that patients with chronic kidney disease can cause olfactory defects and easily lead to malnutrition. A recent new study has discovered a new function of theophylline, which can significantly improve the olfactory defects and malnutrition in patients with kidney disease.

Odor recognition is thought to reflect the central olfactory processing pathway and function of the human body, and its recognition process occurs in epithelial olfactory sensory neurons. The sense of smell plays a vital role in determining the taste of food. The sensitivity of olfactory sensation is related to the occurrence of anorexia.

Studies have shown that olfactory defects can lead to anorexia in patients, leading to malnutrition. Previous studies have reported that the olfactory threshold may be affected by chronic kidney disease. The study found that patients with chronic kidney disease can detect an odor normally, but the detection threshold for another odor substance will increase. This led researchers to speculate that olfactory defects in patients with chronic kidney disease occur and are formed by a variety of mixing mechanisms, including odor recognition and odor thresholds. Loss of smell in patients with chronic kidney disease can lead to improper diet, which in turn leads to poor quality of life, poor overall health, and even death in the early stages of the disease.

Olfactory deficits in patients with kidney disease lead to malnutrition, which is one of the main determinants of morbidity and mortality in patients with kidney disease. Olfactory deficits and their impact on the nutritional status of patients with kidney disease have prompted the development of new interventions to address the complications of malnutrition in this population.

A recent new study found that the use of the inhaled asthma drug, theophylline, can improve the sense of smell in patients with renal failure. 71% of patients with chronic kidney disease who participated in this clinical trial had an increased odor recognition score. This proves that intranasal administration of theophylline can significantly improve the odor recognition ability. The results of this clinical trial suggest that theophylline can be administered nasally to correct scent recognition defects associated with kidney disease. The possible pharmacological mechanism is that theophylline acts by mediating peripheral olfactory processing.

summary

The main effects of theophylline include relaxing the bronchial smooth muscle, increasing myocardial contractility and efficiency, increasing blood pressure and renal blood flow, and having an anti-inflammatory effect. The olfactory threshold may be affected by chronic kidney disease. A recent new study found that the new function of the old drug theophylline can improve the olfactory defects and malnutrition in patients with kidney disease. However, its clinical use may be subject to some restrictions. Because of the complex interactions between theophylline and many drugs, and there is a very obvious feature - with a very narrow therapeutic window. Therefore, blood concentration monitoring must be performed in the clinic to avoid the toxicity of drug overdose by directly measuring the serum theophylline concentration. Regardless, we look forward to further clinical trial results to alleviate olfactory defects and malnutrition in patients with kidney disease.
